Family Entry Form


Please fill out this form and click the "Send Entry" button at
the end. This will email the form to me. Fill out one form for
yourself, and additional ones for each member of your immediate
family (children, spouses, parents) and any other relative you
want to include (grandparents, uncles, aunts, grandchildren, etc.) --
or have them do it.

I'll add an entry to the Aldhizer Family Directory for each
form I receive. Also, if you include family tree info (please),
I'll hook you into your branch into mine. If it doesn't
connect into mine, I will create one of your family. Please
note that the items with asterisks are required, but
all other information is optional (the more information
the better, of course, but the only information that will
be open is the web address.  I DO NOT intend on
posting any personal information, (addresses, phone
numbers) on the site. I'll keep those for myself, so that
I may contact you if I have any questions in the future.
